tbldeleted study should show which process deleted the study and have room for comments

HCA has requested adding a column to tbldeleted study to show which process deleted the study (ILM, Console, Dicom, WCF etc). The additional ask from Tracy is to be able to add a comment. Currently there is a popul in the console that requires the user to type deleteme. This could be modified to save additional text to a column in tbldeleted study for studies deleted from the console.
