Additional check if an approved report already exist before approving

Under very special timing circumstances it is possible that the system ends up with 2 approved reports. (1 done in eRAD and the other through interface). 2 entries in UBERGEBNIS table causes the reports to not be visible in the application anymore...

The field "Writer" should be set with the user who is actual logged in (RIS) and who saves the report first with status "written".
